Treatment of hypertension and chronic stable angina pectoris. Bisoprolol is also used in appropriately selected patients with stable chronic heart failure, depending on formulation and approved indication.
Adult: For hypertension and angina, treatment is generally initiated at a low once-daily dose and adjusted according to clinical response.
A commonly used dose is 5 mg once daily, which may be increased to 10 mg once daily when required.
For chronic heart failure, treatment requires gradual dose titration under medical supervision.
Dose adjustment may be required in patients with severe renal or hepatic impairment.
Each tablet contains:
Bisoprolol Fumarate ........ 5mg.
Bisoprolol is a highly selective β₁-adrenoceptor blocking agent with little affinity for β₂ receptors at therapeutic doses.
Bisoprolol reduces the effects of sympathetic stimulation on the heart by selectively blocking cardiac β₁-adrenoceptors. It decreases heart rate, myocardial contractility and cardiac output, thereby reducing blood pressure and myocardial oxygen demand.
Bisoprolol is well absorbed following oral administration and has high bioavailability. Peak plasma concentrations generally occur within a few hours.
It has an elimination half-life of approximately 10–12 hours, supporting once-daily administration. Elimination occurs through both hepatic metabolism and renal excretion.
Contraindicated in patients with acute heart failure or episodes of decompensated heart failure requiring intravenous inotropic therapy, cardiogenic shock, significant second- or third-degree AV block without a pacemaker, sick sinus syndrome, symptomatic bradycardia, symptomatic hypotension, and hyp...
Use with caution in patients with bronchospastic disease, diabetes mellitus, peripheral arterial disease, thyroid disorders and renal or hepatic impairment.
Treatment should not be discontinued abruptly, particularly in patients with ischaemic heart disease.
Adverse effects may include bradycardia, dizziness, headache, fatigue, cold extremities, hypotension, nausea, vomiting, diarrhoea and constipation.
Overdose may result in severe bradycardia, hypotension, bronchospasm, acute cardiac failure and hypoglycaemia. Immediate medical management and supportive treatment are required.
Interactions may occur with calcium-channel blockers such as verapamil and diltiazem, antiarrhythmics, centrally acting antihypertensive agents and other medicines that reduce heart rate or blood pressure.
